Registration 2013 – 2014 Forms
To register for Religious Education classes print and read the Faith Formation Program Guide. Print and completely fill out the Registration Form, Family Covenant, Medical Form and Time, Talent & Treasure Forms below. All 4 forms must be completed and turn in with registration fee before the registration can be processed. You will be notified if your registration is incomplete. Registration begins on Thursday August 1st, 2013 – completed registration forms and fee can be mailed in or dropped off at the Rectory office M-F 9-1pm.
No Registrations will be accepted in the Faith Formation Office.
